Add Additional Value to Your Educational Activities

Established in 2021, the Louisiana AAP institute for Professional Education (LIPE) has provided CME credit for the educational activities of non-accredited organizations as well as all educational activities of the Louisiana AAP. We work with a variety of organizations to offer both one time and ongoing activities.

Our joint provider partners include specialty societies, hospitals, academic medical centers, government agencies, and non-profit organizations.

In order to be considered for joint providership, you must meet each of the following requirements:

• The target audience should be primarily physicians
• Your organization does not meet the definition of an ACCME defined commercial interest
• The activity or activities which are seeking joint providership must not have already occurred

CME Forms Required for Accreditation

If your activity is approved, a number of CME forms are required for accreditation by the Louisiana AAP Institute for Professional Education.

Please submit all relevant forms at least two weeks (14 days) before the start of the education activity.

Speaker Forms

  • Speaker Information and Disclosure Combination Form

     Each speaker or presenter must submit the Speaker Information and Disclosure Combination Form. This form is valid for 1 year from the date of submission.

  • Activity Planning Form

    For each presentation or session, one speaker must submit the Activity Planning Form.

  • Planner Disclosure Form

    Each planner, activity manager, content contributor, etc. must submit a Disclosure Form. Disclosure forms should be submitted ahead of beginning their role in the activity if possible. This form is valid for 1 year from date of submission.

  • Biography Update

    Please use this form only to update your Biography if there have been changes. If you are a new speaker or your last disclosure with the LA AAP was submitted over 1 year ago, please submit your full speaker information and disclosure form at the link above.

  • Presentation Submission Form

    At least 1 Week before your activity, submit a close-to-final draft for review by education staff in the Presentation Submission Form.

  • Evaluation of Learning Questions Form

    If your activity planner has requested that you submit questions for ABP MOC Part 2 or any continuous credit/maintenance of credit program, please submit three multiple choice questions in the Evaluation of Learning Questions Form.

Joint Provider Forms
