CME Speakers

Speaker Forms

Any speaker participating in an event accredited by the Louisiana AAP and the Louisiana Institute for Professional Education must complete the following forms.

Speaker Information and Disclosure Combination Form (required for speakers)

Activity Planning Form (required – 1 per presentation)

Disclosure Form (required for planners)

Biography (if applicable)

MOC Part 2 Questions (if applicable)

PowerPoint Presentation – Email final presentation (and include a disclosure slide) to the Education Department at CME@laaap.org – Due one week before activity
